ahm believes that quality health insurance should be more than just providing hospital cover or paying for things like new glasses and contact lenses. That's why we've been helping our policy holders look after their health for over a decade through our health management programs.

Health Coaching
Health coaching is a six month support program to help you improve or maintain your health or manage a condition to prevent chronic disease. This program involves a series of telephone calls made by qualified clinicians including dietitians, exercise physiologists and occupational therapists.
- It helps you to make the changes needed to improve your health through motivation and behavioural change
- You’ll be provided with information specific to your health goals and needs
- You can use ahm’s online diary for tracking and setting goals and access a library of health features and recipes.
Services include:
- Consultation to set health goals
- Over the phone support.
Hospital Substitute services
Sometimes a hospital stay can be avoided or your time in hospital reduced if support services are in place in your home. With the permission of your treating doctor, you can elect to recover at home rather than in hospital. Our health consultants tailor a package of care services to assist your recovery and ahm pays 100% of the cost for services delivered when part of an ahm program or when provided by an ahm recognised provider. These providers have a contract with ahm and will bill ahm directly for services.
ahm’s Total Health for Hospital offers two components;
- Assisted discharge for a home recovery
- Alternative to a hospital admission

Chronic disease management programs
For those with a chronic or complex condition who need extra support, ahm’s Total Care program can help improve your quality of life and provide assistance to your carers if you have them. This program can assist policy holders who have conditions including:
- Circulatory or digestive system disease
- Diabetes
- Chronic pain
- Neurological disease
- Cancer/palliative care
- Respiratory disease
- Multiple medical conditions.
The Total Care Program is a confidential, telephone-based program staffed by registered nurses and health consultants trained to help you:
- better manage your illness
- feel more in control
- make informed choices about your health.
Once you enrol, we’ll work with you to develop personal health goals and contact you regularly over the next 12 months to offer support and assistance. You’ll also be able to call us during business hours, Monday to Friday if required and we’ll mail additional support materials to your home from time to time.
ahm's Total Health for Pregnancy*
This program is for new and expecting mothers. ahm’s Pregnancy Support program will give you extra help and information throughout your pregnancy up until your child is 12 months old. Our midwives contact you at key dates during your pregnancy to help you with questions about your pregnancy, diet, exercise or baby care. You'll also have access to a dedicated phone line and email for any unexpected questions. ahm’s comprehensive web site features articles and health content customised for pregnancy and parenting, plus, a pregnancy calendar that shows you what to expect week by week.

It's a great way to improve your health - pick the health issue you want to address, set your target and your action plan - all on your terms. If your profile shows your health could benefit from extra support we'll contact you to identify the steps you need to achieve your goal, and coach you along the way.

Important: Some of the programs are subject to eligibility, hospital participation and your doctor’s approval. There are no waiting periods for the disease prevention and chronic disease management programs, normal waiting periods apply to other programs. ahm pays 100% of the cost for all eligible programs and services. You must be 18 years or older to participate in these programs.
