ahm's Chronic and Complex Care Program
ahm’s Chronic and Complex Care Program offers extra support for complex health issues such as cancer, diabetes, neurological disease, chronic pain or multiple medical conditions. While your doctor focuses on the most pressing issues, we can help you manage other lifestyle factors that contribute to your wellbeing like stress, diet or exercise so you can feel more in control of your life.
Professional advice and support
While on this 12-month program, you’ll be contacted regularly by an experienced health professional such as a Registered Nurse, Dietitian, Psychologist or Exercise Physiologist. They will give you information about your illness or condition and motivate you to better manage it and other lifestyle factors.
- You’ll receive a written plan with agreed goals and activities developed by a qualified clinician
- It’ll help you make changes needed to improve or manage your condition through motivation and behavioural changes
- You’ll be provided with information specific to your health goals and needs
- You can use ahm’s online health diary for setting and tracking goals
- Access our online Health Library for articles, tips and recipes.
Services include:
- Telephone support from a qualified Health Consultant
- Co-ordination of services to manage the condition
- Phone in and email service
- Educational material and online resources
- Health Coaching.
No cost to you
This program is fully covered on the following hospital covers:
- Basic Hospital
- Essential Hospital Level 5
- Family Hospital Level 5
- Top Hospital, Top Hospital Level 5 and Top Hospital Level 8
The cost of the program is covered under your policy with ahm, so you don’t have anything to pay!
Who can enrol?
If you’ve been diagnosed as having a chronic illness or you have been diagnosed as having multiple risk factors for chronic disease, call us on 134 246 or enrol online. You must be aged 18 or over to enrol.
