Total Care Program
The Total Care Program provides telephone assistance for people with chronic or complex health problems requiring extra support. The program encourages participants to improve their health through the identification of lifestyle risks, goal setting, and improved self-management of their condition.
The program aims to improve the health of participants in order to reduce unplanned hospital admission and reliance on the health care system. The Total Care Program endeavours to deliver improved quality of life and reduce the burden on carers by assisting clients in:
- improving self management,
- taking responsibility for lifestyle health risks where possible, and
- making appropriate use of community services and support networks.
