Take control of your health with Total Care – a telephone based program offering you extra support for your complex health needs.

While your doctor focuses on the most pressing issues to do with your illness, they may not always have time to help you with all the other factors that contribute to your wellbeing. Things like how you deal with stress, the food you eat and your weight.

That’s where Total Care comes in.

About Total Care

ahm Total Care is a confidential telephone based program that offers extra support and advice for people with complex health needs. Our goal is to help you improve your overall health and quality of life, and to make things a bit easier for everyone involved.

This program has been designed to assist you if you have a condition including:

Diabetes
Chronic pain
Neurological disease
Cancer/palliative care
Respiratory disease
Multiple medical conditions

Our registered nurses and health consultants can help you

better manage your condition
feel more in control
make more informed choices about your health

How it works

Once you enrol, we'll work with you to develop personal health goals and contact you regularly over the next 12 months to offer support and assistance. You'll also be able to call us during business hours, and we'll mail you additional support materials.

Privacy

Rest assured your privacy is protected at all times as only the staff delivering the program will see your health information.

And there's no cost to you!

The great news is that your ahm hospital or hospital and extras cover provides access to this program at no cost to you. We cover the full cost of services provided.
