Assisted discharge (home recovery)

With this program, not only do you get to leave hospital earlier, but we also help by providing you with a range of support services at home. To qualify, your stay must be in a participating hospital and must be at least 2 nights.

How can we help you?

Our clinical staff can work with your doctor and hospital staff to provide a range of support services to make your hospitalisation and recovery easier and more comfortable. These services can include:

  • General and specialist nursing services
  • Personal care
  • Domestic services
  • Transport
  • The use of assisted care facilities, as required

Who can enrol on this program?

You are eligible to participate in the Assisted Discharge program if:

  • Your hospital stay has to be more than two nights or you’ve given birth at one of our program participating hospitals
  • You are over 18 years old
  • You have eligible hospital cover with ahm and you've served your waiting periods
  • Your doctor and the hospital staff must consider it appropriate for you to leave hospital early.

Maternity options
If you are pregnant, our Maternity Options allows you to leave hospital early after the birth of your baby, if clinically appropriate. If eligible, you will also receive a range of support services in your own home. To qualify, you must have an anticipated stay in a participating hospital for at least 3 nights or less for a normal birth or 4 nights or less for a caesarean birth.

Mary's story

Mary* lived alone and knew she would need assistance after her operation as her relatives lived interstate. With her doctor’s consent, Mary received co-ordinated home services to help with her recovery allowing her to leave hospital earlier. This included visits from a personal carer to help with showering, some domestic cleaning and putting on anti-embolic stockings.
