Terms and conditions of entry

  1. Information on prizes and how to enter form part of these Terms and Conditions of Entry. Entry in this promotion is deemed acceptance of these Terms and Conditions of Entry.
  2. Entry is only open to ahm private health insurance (PHI) members (as defined by the ahm Fund Rules, which are available on the ahm web site):

    3. a. who are over the age of 18 years as at 30 June 2009;

    b. whose policy is current and financial for the entire Promotion Period;

    c. who enter the ahm website at www.ahm.com.au/online-signup, and register for ahm's online services during the Promotion Period.

  3. Eligible entrants may only register for ahm's online services once during the Promotion Period and will receive one entry into the prize draw.
  4. There is a limit of 1 entry per ahm private health insurance policy. Where entrants hold ahm hospital and extras cover, the covers together are considered one PHI policy for the purposes of this promotion.
  5. Employees and immediate families of the Promoter, associated companies, suppliers and agencies, are ineligible to enter the promotion.
  6. The Promoter's decision in relation to any aspect of the promotion is final and binding on each person who enters.
  7. The use of any automated software or any other mechanical or electronic means that permits the entrant automatically to enter repeatedly is prohibited. If the Promoter reasonably believes that an entrant is using or has used any such software or other means, the Promoter may disqualify that entrant without notice.
  8. The Promoter, in its absolute discretion, reserves the right to verify the eligibility and validity of all entries and to disqualify any entrant who tampers with the entry process, or who submits an entry that is not in accordance with these Terms and Conditions of Entry.
  9. This promotion commences 9.00am EST Tuesday June 15 2010 and closes at 12noon EST Friday 13 August 2010 ("The Promotion Period").
  10. On Thursday 19 August 2010 at 12 noon EST, an electronic random draw of all correct and valid entries will take place at the offices of the Promoter, 77 Market Street, Wollongong NSW 2500.
  11. One winner will be awarded first prize of a $150 The Athlete's Foot Gift Card valued at $150 and 20 second place winners will be awarded a Rebel Sport Gift Card valued at $30.00 each.
  12. The Athlete's Foot Gift Card is subject to The Athlete's Foot own terms and conditions.
  13. The Rebel Sport Gift Card is subject to Rebel Sport's own terms and conditions.
  14. The winners will be contacted by mail and will be couriered their prize within 28 days of the draw date. The Promoter does not take any responsibility for prizes being lost, damaged or destroyed in transit.
  15. The winners awarded prizes over the value of $100 will be advertised in The Australian newspaper on the following date; Monday 30 August 2010.
  16. Limit of one prize per ahm PHI policy for this promotion.
  17. Total prize pool is $750.00.
  18. The prize is not redeemable for cash, is not exchangeable or transferable and cannot be taken in conjunction with any other offer.
  19. Subject to written directions from State Regulatory bodies if for any reason this promotion is not capable of running as planned including but not limited to technical failures, unauthorised intervention, fraud or any other causes beyond the control of the Promoter which corrupts or affect the administration, security, fairness, integrity or proper conduct of this promotion, the Promoter reserves the right in its sole discretion to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the promotion.
  20. The Promoter excludes all liability (including negligence) for any personal injury, or any loss or damage (including loss of opportunity); whether direct, indirect, special or consequential arising in any way out of this promotion, except for any liability which cannot be excluded by law.
  21. The Promoter collects entrants’ personal information in order to inform them of their prize. Entry is conditional upon providing this information. Entrants further consent to the disclosure by the Promoter of their personal information to the related entities of the Promoter and to any third party services provider assisting the Promoter in the conduct of this promotion. All personal details of the entrants will be stored at the office of the Promoter. As per the relevant privacy legislation, a request to access, update or correct any information should be directed to the Promoter at their address set out above.
  22. The entrant consents to the Promoter using the entrant’s name, likeness, and image in the event that they are a winner in any media for an unlimited period without remuneration for the purposes of promoting products and services supplied by the Promoter, marketing and publicity.
  23. The Promoter may conduct such draws as are necessary if the prize remains unclaimed three months after the draw in accordance with the appropriate legislation. If necessary, a draw will be conducted at EST 12.00 noon on Monday 22 November 2010 at the Promoter’s offices, 77 Market Street, Wollongong NSW 2500.
