ahm’s Chronic and Complex Care program offers extra support for complex health issues such as cancer, neurological disease and chronic pain. If you feel like your condition is starting to control your life, this program is for you.

No cost to you

This program is fully covered on most of ahm’s Hospital covers. The cost of the program is covered under your policy with ahm, so you don’t have anything to pay!

Who can enrol?

If you’ve been diagnosed as having a chronic illness or you have been diagnosed as having multiple risk factors for chronic disease, call us on 134 246 or enrol below. You must be aged 18 or over to enrol.

A Health Consultant will phone you within two weeks to set up your first consultation.

I hereby declare that I am over 18 years old and a financial member of ahm Health Insurance with an eligible Hospital cover (I understand that some members are not eligible for this program). I am enrolling on behalf of myself and not another member.
I hereby provide consent to participate in the program. I understand and accept that ahm has arranged for the program to be delivered and managed by a related Medibank group company, Medibank Health Solutions Telehealth Pty Limited (Telehealth) and agree to the collection, use and disclosure of my personal including sensitive information in accordance with Telehealth’s privacy statement.
