ahm Total Health Program Promotion - Terms and Conditions

  1. Information on prizes and how to enter form part of these conditions of entry. Entry is only open to current and financial ahm private health insurance (PHI) policy holders and all customers on the policy who are over the age of 18 years as at 30 June 2008. Policy holders must be financial with ahm for equal to or more than 12 months as at the draw date.
  2. Employees and immediate families of the Promoter, associated companies and agencies, are ineligible to enter the promotion.
  3. By participating, entrants agree to be bound by these conditions. The Promoter’s decision in relation to any aspect of the promotion is final and binding on each person who enters.
  4. The Promoter, in its absolute discretion, reserves the right to verify the eligibility and validity of all entries and to disqualify any entrant who tampers with the entry process, including but not limited to, techniques designed to avoid payment, or who submits an entry that is not in accordance with these Conditions of Entry.
  5. This offer is valid from 9am EST Wednesday 9 July 2008 until 11.59pm EST Tuesday 30 June 2009.
  6. From 9 July 2008, every current and financial ahm PHI policy holder and customers who (after meeting program eligibility conditions) enrols or re-enrols in any ahm Total Health Program – Health Coaching, Pregnancy Support, Hospital Substitute or Total Care - will be automatically entered into the monthly draw.
  7. On the third Monday of each month at 12 noon, an electronic draw of all valid entries will take place at the offices of the Promoter, 77 Market Street, Wollongong NSW 2500. Draw promotion dates are: Monday 18 August 2008, Monday 15 September 2008, Monday 20 October 2008, Monday 17 November 2008, Monday 15 December 2008, Monday 19 January 2009, Monday 16 February 2009, Monday 16 March 2009, Monday 20 April 2009, Monday 18 May 2009, Monday 15 June 2009 and the last draw will be at 12 noon on the first Monday of July which is Monday 6 July 2009.
  8. Ten winners will be awarded every month with one month’s free premium based on their current PHI cover at the time and date of the draw up to a maximum value of $484.20. One month’s free premium will include the winner’s Federal Government Rebate entitlement.
  9. Winners will be posted a cheque to the value of their prize within 28 days of the draw date.
  10. Winners awarded prizes over the value of $100 will be advertised in The Australian newspaper on the following dates; Monday 25 August 2008, Monday 22 September 2008, Monday 27 October 2008, Monday 24 November 2008, Monday 22 December 2008, Tuesday 27 January 2009, Monday 23 February 2009, Monday 23 March 2009, Monday 27 April 2009, Monday 25 May 2009, Monday 22 June 2009 and Monday 13 July 2009.
  11. Policy holders and customers who are unsuccessful in the monthly draw will be automatically re-entered into each following monthly draw, providing they remain enrolled on an ahm Total Health program and continue to meet the eligibility criteria set out in point 6.
  12. Policy holders and customers who are already enrolled on an ahm Total Health program prior to 9 July 2008 are ineligible to enter the competition unless they meet the eligibility criteria set out in Condition 6 which is enrolling or re-enrolling on a program during the promotion period.
  13. Limit of one prize per ahm PHI policy.
  14. Total prize pool is up to the value of $58,104.00.
  15. This offer cannot be taken in conjunction with any other offer.
  16. If for any reason this promotion is not capable of running as planned including but not limited to technical failures, unauthorised intervention, fraud or any other causes beyond the control of the Promoter which corrupts or affect the administration, security, fairness, integrity or proper conduct of this promotion, the Promoter reserves the right in its sole discretion to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the promotion.
  17. The Promoter shall not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) or for personal injury which is suffered or sustained, as a result of receiving the prize, except for any liability which cannot be excluded by law.
  18. The Promoter collects entrants’ personal information in order to enrol them on an ahm Total Health program. If the information requested is not provided, the entrant may not be able to enrol on a program and participate in the promotion. Entrants further consent to the disclosure by the Promoter of their personal information to the related entities of the Promoter and to any third party services provider assisting the Promoter in the conduct of this promotion. All personal details of the entrants will be stored at the office of the Promoter. As per the relevant privacy legislation, a request to access, update or correct any information should be directed to the Promoter at their address set out above.
  19. The Promoter may conduct such draws as are necessary if the prize remains unclaimed three months after the draw in accordance with the appropriate legislation. If necessary, a draw will be conducted at 12.00 noon on Monday 17 November 2008, Monday 15 December 2008, Monday 19 January 2009, Monday 16 February 2009, Monday 16 March 2009, Monday 20 April 2009, Monday 18 May 2009, Monday 15 June 2009, Monday 20 July 2009, Monday 17 August 2009, Monday 21 September 2009 and Monday 19 October 2009 at the Promoter’s offices, 77 Market Street, Wollongong NSW 2500.
